Who's eligible for individual insurance plans
With SuperMed One – Ohio Farm Bureau Dental plans are available to all ages and maybe purchased in conjunction with a medical plan or as a stand-alone product.
You and any dependents may apply for With SuperMed One – Ohio Farm Bureau insurance as long as you are residents of Ohio at least six months of the year.
Family coverage includes you, your spouse and any unmarried children under the age of 23. Dependent-only coverage is available.

Call a DenteMax dentist to schedule an appointment.
Let the dentist know that you are a Medical Mutual SuperMed One – Ohio Farm Bureau member. Then present your identification card at your first visit.
A claim will be submitted to Medical Mutual for the services you received
