As a SuperMed One – Ohio Farm Bureau Vision member you have annual benefits towards an eyeglass examination and pair of eyeglasses or contact lenses, available through thousands of participating network providers. Reduced benefits are available when you obtain services outside the vision care provider network.

Who's eligible for personal vision care plans
SuperMed One – Ohio Farm Bureau Vision plans are available to all ages and maybe purchased in conjunction with a medical plan or as a stand-alone product.
You and any dependents may apply for SuperMed One – Ohio Farm Bureau insurance as long as you are residents of Ohio at least six months of the year.
Family coverage includes you, your spouse and any unmarried children under the age of 19 (23 for full-time students). Dependent-only coverage is available.
* Please Note: When purchasing only a SuperMed Dental or Vision plan, one year of premium will be required at the time the first billing statement is received.
